Complete and Timely Documentation of Medicare Services

Documentation is the compilation of important facts about the patient’s encounter. Documentation must:
- Contain data to identify the patient.
- Support the reason for the encounter.
- Justify the treatment.
- Document the results of tests or diagnostic procedures.
- Providers complete documentation in a timely manner.
Our Medical Directors feel that complete and timely documentation is important for the patient’s care and the communication between providers.
Purpose of Documentation
Documentation is the link between the service provided and Medicare’s payment. Use the following tips to help avoid errors. Errors are time consuming to research and fix. Additionally, incomplete documentation delays payment for services.
Documentation proves care occurred. It helps Medicare with:
- A timeline of patient care management
- An aid in legal affairs
- Quality review
Provider’s use documentation to:
- Track patient care
- Plan for future patient care
- Support payment from insurance
Medicare uses documentation to:
- Support financial reimbursement
- Public health statistics
- Detect potential fraud and abuse
Documentation Components
Documentation is complete when it contains:
- The patient’s information
- Patient’s name
- Date of birth
- Sex
- Medicare Beneficiary Identifier (MBI)
- Date and time
- Include the calendar days when the service goes overnight
- Include multiple times for visits on the same day
- Reason for the service
- Include the patient complaint
- Provider observations
- Rationale for ordering tests
- Items or services provided
- Document all portions of the patient encounter
- Number of services or items
- Time for procedure services requiring it for coding
- Provider of service
- The number of times an item or service occurred
- Service location
- Physical location
- Nine-digit ZIP code
Complete Documentation
Medical record documentation must:
- Identify the patient by name and date of birth.
- Both elements should match the Medicare system.
- Include an explanation if information does not match the Medicare system.
- Identify the performing provider by name and medical credentials.
- Identify the ordering or referring provider by name and medical credentials.
- Identify the date of service.
- Contain the patient’s chief complaint or reason for today’s encounter.
- Support service(s) meets medical necessity.
- Support services billed according to Medicare guidelines.
- Support billed diagnosis code.
- Support billed procedure code.
- Contain all provider signatures.
- Contain the patient’s signature, including:
- One-time signature allowing Medicare billing.
- Be legible to complete a fair review.

Legible Documentation
Consider the following to ensure your documentation is legible:
- Defining acronyms and abbreviations
- Not standard in the medical world.
- With more than one mean in the standard use.
- Legible provider signatures
- Send a signature log or attestation statement for illegible signatures.
- Send an attestation statement for a missing provider signature.
- Transcribing illegible documentation
When necessary, the billing provider must obtain supporting documentation from a third party. These parties include:
- Hospital
- Nursing home
- Doctor’s offices
- Any other provider with supporting documentation
Do not refer Medicare contractors to a third party to obtain records.
Electronic Documentation
When using electronic medical records to document, consider the following:
- Can we identify what makes each encounter or service unique?
- Indicate the review of copied information.
- Can we identify the pages are one record?
- Does each page indicate the patient’s identifying information?
- Are required signatures included in the medical record?
Date of Service
Medicare rules require anyone billing Medicare to submit the correct date of service.
The date of service is the date you provide the service. The date of service does not depend on another provider’s date of service.
When billing a claim, there are some exceptions for billing the date of service for certain services. The exceptions include the rules applying to:
- Radiology
- Anatomical pathology
- Surgery services

ZIP Code
CMS requires contactors to determine the payment locality for services paid under:
- The Medicare Physician Fee Schedule (MPFS)
- Anesthesia services
Always include a nine-digit Place of Service (POS) location ZIP code. The ZIP code represents the physical location of the service.
To determine payment locality, we use the ZIP code on the claim identifying where providers preformed the service. The ZIP code submitted must be nine digits.
The CMS ZIP Code file uses the convention of the United States Postal Service (USPS). THE USPS assigns these ZIP codes into counties. In some cases, services rendered in one county have a ZIP code assigned into a different county. This causes a payment issue when the counties have different payment localities with different payment amounts.

We reject claims payable under the MPFS and anesthesia services without the service facility location when rendered in:
- POS home
- POS we consider the patient’s home
Pre-Charting
Medicare recognizes many providers pre-chart to make documenting easier. Pre-charting means the provider or their ancillary staff enter information on the patient chart before seeing the patient. If you use this process, the physician or non-physician must document evidence of:
- Provider reviewed the medical record.
- Clinical updates on patient status to reflect today’s visit.
- The medical necessity and clinical interventions for today’s visit.
Do not copy and paste treatment notes from the patient’s medical record without providing unique identifiers for today’s visit.
Timely Documentation
Medicare expects practitioners to complete the documentation of services when they render the service. This information is in the CMS Internet-Only Manual (IOM), Publication (Pub) 100-08, Medicare Program Integrity Manual, Chapter 3 , Section 3.3.2.5. CMS also indicates practitioners should complete documentation during or as soon as practicable after an encounter. This information is in the IOM, Pub 100-04, Medicare Program Integrity Manual, Chapter 12 , Section 30.6.1.
Following the above instructions maintains accurate medical records and aids in provider communication. The CMS IOM does not provide any specific time for “as soon as practicable.” WPS Government Health Administrators Medical Directors offer a reasonable time frame of 24-48 hours.
A provider must complete the documentation before submitting a claim to Medicare. Medicare does not provide reimbursement for services without supporting documentation. Some providers choose a date once a month or once a quarter to document the services. This decision delays Medicare payments and can affect patient care.
You do not have to wait on the provider’s signature prior to submitting the claim. The provider may sign the documentation after claim submission. Medicare considers notes "incomplete" when missing the provider signature.
Addendums
WPS Government Health Administrators recognizes that at the time-of-service the following may prevent providers from documenting the service:
- Clarification
- Error correction
- Missing information
- Unplanned circumstances
When providers cannot document at the time of service, add a note to the record explaining the delay. Below we provide examples. This not an all-inclusive list.
- Acceptable circumstances
- A provider went into labor and has not returned to work yet.
- A provider was in a car accident and unable to return to work before they dictated the record.
- Not Acceptable Circumstances
- The provider went on vacation and did not complete dictation.
- The provider forgot to complete the record.
Add an addendum stating the errors or the missing information. We recognize late entries to clarify the original written explanations. Never alter an original record. The addition should not:
- Prove the medical necessity.
- Prove the fact the provider performed a service.
Include the following information in the addendum:
- The date the provider added it (not the original date of the record).
- Legible signature of the person making the entry.
- Legible provider initials and credentials.
A best practice is to also include the time of the entry.
Documentation Requests
Submit all documentation requested, not just the documentation in your office. Do not refer Medicare to a different provider or facility for supporting documentation. When you submit a Medicare claim, you take the responsibility to gather all documentation to support your claim.
Remember, accurate and timely completion of medical records is part of the provider’s responsibility to the patient and Medicare.
